VitalCaring Group is seeking a Field-Based Home Health RN to lead patient care in their homes, exercising clinical autonomy and delivering high-impact nursing. You will manage plans of care, coordinate with multidisciplinary teams, and drive outcomes that reduce hospitalizations while supporting patient independence.

This role emphasizes critical thinking, independent decision-making, and strong communication with patients, families, and care teams.

Qualificações

• Active RN license (or compact eligibility) and CPR certification.
• Minimum 1 year RN experience; Home Health preferred.
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Proficient with EMR systems; valid driver’s license.

Responsabilidades

• Conduct in-home assessments evaluating clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management, treatments, and interventions.
• Serve as the case manager coordinating care across therapists, aides, and physicians.
• Monitor patient progress and adjust plans based on changing conditions.
• Educate patients and families on disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation.
