Job summary

VitalCaring Group is seeking an experienced Home Health RN to lead plans of care for patients in their homes across a defined territory. You will assess, plan, and deliver skilled nursing care while driving outcomes and reducing hospitalizations.

You will manage care transitions, educate families, and collaborate with physicians, therapists, and aides in a fast-paced, autonomous role that emphasizes quality and independence.

Qualifications

• Active RN license and CPR certification required.
• Graduate from an accredited nursing program; BSN preferred.
• Strong clinical judgment and ability to think independently.

Responsibilities

• Conduct comprehensive in-home assessments and evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management and interventions.
• Serve as case manager coordinating across therapists, aides, and physicians.
• Monitor patient progress and adjust care plans based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
