Job summary

Denova Collaborative Health LLC is seeking a Family Nurse Practitioner for a part-time, onsite role in Arizona. You will deliver autonomous, comprehensive primary care within an integrated care model across life stages.

You will conduct thorough assessments, develop treatment plans, and prescribe medications while collaborating with a multidisciplinary team. Requires Master’s in nursing, AZ NP license, FNP board certification (DEA/ANCC), BLS, and 1–3 years of relevant experience.

Qualifications

Master’s degree in nursing.
Current Arizona Nurse Practitioner licensure.
Board certified Family Nurse Practitioner with DEA and ANCC certifications.
Experience using electronic medical records.
1–3 years of Family Practice/Primary Care NP experience.
Strong clinical assessment, communication, and time management skills.
Ability to work independently while collaborating within a multidisciplinary team.
Current BLS and a valid Arizona Fingerprint Clearance card.
