Denova Collaborative Health LLC is seeking a Family Nurse Practitioner to support an integrated primary care and behavioral health model in Arizona. This part-time, on-site role involves 2 ten-hour shifts weekly (7:00 AM–5:00 PM) with at least one day on Monday or Friday.

We offer licensing and CME reimbursements, no-cost credentialing, and comprehensive malpractice coverage. You will lead care, develop treatment plans, and collaborate with a multidisciplinary team while maintaining strong

Formación

• Master’s degree in nursing is required.
• Arizona NP licensure in good standing is required.
• Board certified Family Nurse Practitioner with current DEA and ANCC certifications.
• Experience using electronic medical records is required.
• Current BLS certification and Arizona Fingerprint Clearance card are required.
• 1–3 years of Family Practice or Primary Care NP experience.
• Integrated care setting experience is a plus.
• Strong clinical assessment, communication and time management skills.
• Ability to work independently while collaborating in a multidisciplinary team.
