Job summary

The Somerset NHS Foundation Trust is seeking a Community Nurse to join our friendly multi-disciplinary team. You will provide autonomous, holistic care, assess complex and simple needs, and develop and evaluate care plans with support from the Team Leader and Clinical Lead.

You will manage a caseload across the out of hospital model, liaising with Primary Care Networks and Neighbourhood Teams. A driving licence and access to a car are essential, and flexible hours within a 8am–10pm roster are

• NMC registered nurse with post-registration training.
• Commitment to evidence-based practice and professional development.
