Mersey Care NHS Foundation Trust is recruiting a Nursing Associate for community nursing. The role involves travel across multiple sites within the Mersey Care catchment, with the ability to work without direct supervision under the guidance of registered nurses.

You will provide holistic, person-centred care across the lifespan in home and community settings, supporting assessments, care planning, and delivery while adhering to NMC standards and Mersey Care policies.

Responsabilités

• Deliver safe and effective care to patients in line with individual care plans and clinical guidelines.
• Support registered nurses in the assessment and monitoring of patients, escalating concerns appropriately.
• Administer medications and treatments within scope of practice and in accordance with NMC standards.
• Promote health and wellbeing through education, advice, and support to patients and carers.
• Maintain accurate and timely clinical records using electronic systems.
• Participate in clinical audits, quality improvement initiatives, and service development.
• Work collaboratively with other health and social care professionals to ensure coordinated care.
• Uphold professional standards and contribute to a culture of continuous learning and development.
• Supervise and support junior staff and students as appropriate.
• Adhere to Mersey Care’s values and policies, including safeguarding, infection control, and equality and diversity.
