Qualifications

• Bachelor’s degree or higher required with eligibility for QIDP certification.
• Experience providing case management for individuals with intellectual/developmental disabilities desirable.

Responsibilities

• Manage individual service plans and ensure compliance with waiver requirements.
• Coordinate and monitor delivery of waivered services and document appropriately.
• Develop social service evaluations and care recommendations.
• Oversee continuity of care and waiver benefits for assigned individuals.
• Assist individuals and families in accessing social services and benefits.
