Job summary

State of Connecticut DSS is seeking five Developmental Services Case Managers for the Autism Waiver Services Division. Positions involve serving caseloads across multiple regions with a hybrid work arrangement in Hartford, CT.

Strong focus on Person-Centered Planning and facilitating access to Medicaid and waiver benefits. Ideal candidates will have experience coordinating waivered services, developing service plans, and supporting individuals and families.

Qualifications

• Experience identifying and accessing resources for individuals with Autism.
• Knowledge of Medicaid and HCBS Autism Waiver eligibility.
• Experience with Person-Centered Planning and self-directed supports.

Responsibilities

• Coordinate waivered services to ensure federal reimbursement is met.
• Develop and monitor individual service plans and waiver benefits.
• Provide information and support to individuals and families to access social services and benefits.
• Collaborate with cross-functional teams to ensure service delivery and compliance.
