Community Horizons is seeking a caregiver and companion to work directly with the parent to coordinate the day’s plan, focusing on structure, skill building, and engagement.

You will provide companionship, independence coaching, and help with daily routines, personal care, meal prep, and homeschooling support. A flexible, active personality and the ability to adapt to the individual’s needs are essential, and the home is cat-friendly.

Qualifications

• Valid driver's license and reliable transportation.
• Ability to pass Wisconsin background check.
• Prior experience working with children or individuals with special needs preferred.
• The right personality: flexible, active, and ready to coach rather than supervise.
• This is a cat-friendly home.

Responsibilities

• Companionship: Be a genuine friend and engage in activities.
• Independence coaching: Model, teach, and assist with independent living skills and ADLs.
• Daily routines & personal care: Assist with morning routines, mobility, and personal care.
