Job summary

EMS of Virginia is seeking a Qualified Mental Health Professional (QMHP) to provide Life Skills Coordination in our Pathway Partners program. The role supports clients in the community, including home visits, treatment planning, and linking to community resources in the greater Richmond area.

The ideal candidate is QMHP-registered, has experience with autism spectrum or neurodivergence, and can participate in supervision and team meetings.

Qualifications

• QMHP registered with the Board of Counseling is required.
• Experience with autism spectrum or neurodivergence is highly desirable.
• Strong skills in community-based engagement and case management are essential.

Responsibilities

• See clients in the community, including in the client's home.
• Provide 1-1 mental health counseling related to goals.
• Follow Life Skills modules and develop active treatment planning.
• Transport clients to appointments and link to resources.
• Perform case management to meet basic needs and complete service plans.
• Prepare monthly reports and progress notes for sessions.
• Document information for authorization of services and participate in supervision.
