Qualifications

• Bachelor's degree in Social Work, Psychology, or related human services required.
• Excellent interpersonal, communication, and relationship-building skills required.
• Ability to manage multiple priorities while maintaining accuracy and attention to detail.
• Proficiency with Microsoft Office and EMR/documentation systems."

Responsibilities

• Guide families through the autism screening and assessment process from initial contact through service coordination.
• Facilitate autism screenings and provide education and support to families regarding screening outcomes and next steps.
• Serve as the primary point of contact for assigned families, providing ongoing case management, advocacy, and resource navigation.
• Develop individualized service plans and coordinate referrals to community resources and Easterseals programs.
• Collaborate with physicians, therapists, community partners, and multidisciplinary team members to ensure quality, person-centered care.
• Educate families on available services, intervention options, benefits, and community supports.
