Job summary

Easterseals Florida is seeking a compassionate Case Manager to guide families through autism screening and assessment, coordinating services and resources from initial contact to ongoing support.

You will work with families, physicians, and partners to ensure person-centered care, education on services, and community outreach initiatives, contributing to better outcomes for children and families.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related human services field is required.
• Excellent interpersonal, communication, and relationship-building skills are essential.
• Ability to manage multiple priorities with accuracy and attention to detail.
• Knowledge of autism and health insurance, local resources, and care coordination is preferred.

Responsibilities

• Guide families through the autism screening and assessment process from initial contact through service coordination
• Facilitate autism screenings and provide education and support to families regarding screening outcomes and next steps
• Serve as the primary point of contact for assigned families, providing ongoing case management, advocacy, and resource navigation
• Develop individualized service plans and coordinate referrals to community resources and Easterseals programs
• Collaborate with physicians, therapists, community partners, and multidisciplinary team members to ensure quality, person-centered care
• Educate families on available services, intervention options, benefits, and community supports
• Participate in community outreach activities, educational events, and autism awareness initiatives
• Assist in developing educational resources, workshops, and materials for families and community partners
• Maintain accurate client records, service documentation, and program data while ensuring compliance with reporting requirements and established timelines
• Support grant reporting, quality assurance activities, and program evaluation through data tracking, record review, and administrative support for Autism Services leadership

Location: Daytona Beach, Florida

Salary: $22.00 to $25.00 per hour (based on experience)

Status: Full-time, 40 hours per week

Schedule: Typically Monday - Friday, 8:00 a.m. - 5:00 pm, with flexibility for some weekend and evenings

What We're Looking For

• Bachelor's degree in Social Work, Psychology, or related human services field
• Minimum one (1) year of experience in social work, case management, or human services
• Knowledge of Autism Spectrum Disorder (ASD) required
• Knowledge about health insurance, local human service community resources, family support services, and care coordination
• Excellent interpersonal, communication, and relationship-building skills
• Ability to manage multiple priorities while maintaining accuracy and attention to detail
• Ability to work collaboratively with families, healthcare professionals, educators, and community partners
• Proficiency with Microsoft Office (Word, Excel, PowerPoint, Outlook) and electronic medical record/documentation systems
• Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)
