Job summary

Easterseals Florida, Inc. in Daytona Beach is seeking a compassionate Case Manager to support children and families within the Autism Community Outreach team.

You will guide families through screening and service coordination, connecting them with critical resources and coordinating services. Your role includes developing individualized service plans, advocating for families, and collaborating with healthcare professionals to ensure high-quality, person-centered care, while maintaining records

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• Minimum one year of experience in social work, case management, or human services.
• Knowledge of Autism Spectrum Disorder (ASD) required.
• Proficiency with Microsoft Office and EMR/documentation systems.

Responsibilities

• Guide families through the autism screening and assessment process from initial contact through service coordination.
• Facilitate autism screenings and provide education and support to families regarding screening outcomes and next steps.
• Serve as the primary point of contact for assigned families, providing ongoing case management, advocacy, and resource navigation.
• Develop individualized service plans and coordinate referrals to community resources and Easterseals programs.
• Collaborate with physicians, therapists, community partners, and multidisciplinary team members to ensure quality, person-centered care.
• Educate families on available services, intervention options, benefits, and community supports.
• Maintain accurate client records, service documentation, and program data while ensuring compliance with reporting requirements and established timelines.

Location: Daytona Beach, Florida

Salary: $22.00 to $25.00 per hour (based on experience)

Status: Full-time, 40 hours per week

Schedule: Typically Monday - Friday, 8:00 a.m. - 5:00 p.m., with flexibility for some weekend and evenings

What You'll Do

• Participate in community outreach activities, educational events, and autism awareness initiatives
• Assist in developing educational resources, workshops, and materials for families and community partners
• Maintain accurate client records, service documentation, and program data while ensuring compliance with reporting requirements and established timelines
• Support grant reporting, quality assurance activities, and program evaluation through data tracking, record review, and administrative support for Autism Services leadership

What We're Looking For

• Bachelor's degree in Social Work, Psychology, or related human services field
• Minimum one (1) year of experience in social work, case management, or human services
• Knowledge of Autism Spectrum Disorder (ASD) required
• Knowledge about health insurance, local human service community resources, family support services, and care coordination
• Excellent interpersonal, communication, and relationship-building skills
• Ability to manage multiple priorities while maintaining accuracy and attention to detail
• Ability to work collaboratively with families, healthcare professionals, educators, and community partners
• Proficiency with Microsoft Office (Word, Excel, PowerPoint, Outlook) and electronic medical record/documentation systems
• Ability to meet our approved driver qualifications in order to drive your personal vehicle, as required, for program-related needs (minimum age of 21 and good driving history)
