Job summary

Washington State Department of Social and Health Services Lakeland Village is seeking an Attendant Counselor 2 to provide direct care under RN/LPN supervision. The role includes writing and evaluating training programs and working with an interdisciplinary team to support residents in daily living.

The AC2 will implement individualized plans, monitor residents, assist with transfers, transport to activities, and participate in data collection for care planning.

Qualifications

• Ability to support staff carrying out 1:1 or 2:1 staffing with assaultive residents.
• Understanding of Therapeutic Options training in daily work assignments.
• Ability to receive and follow written instructions accurately.
• Ability to implement individualized programs and services to maintain skills.
• Ability to collect and record data per IHP plans and behavior supports.

Responsibilities

• Takes and records vital signs and measures weight, intake and output.
• Transports clients to appointments and activities on campus.
• Implement care plans and behavioral interventions under supervision.
• Document changes in residents' condition and report to supervisors.
