Job summary

CenterWell Home Health is seeking a Home Health Speech-Language Pathologist to deliver in-home therapy, evaluate patients, and contribute to care plans. You will work under the Clinical Manager to promote rehabilitation and independence in patients’ homes and facilities.

The role requires ASHA CCC, at least six months of SLP experience, an unrestricted license, CPR, and a valid driver’s license with reliable transportation. Full-time position with base pay above the market range.

Qualifications

• Certification by ASHA for Clinical Competence in Speech-Language Pathology or Audiology.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted professional license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing, and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Record changes in patient condition and report to supervisor or physician.
• Provide instruction on alternative communication methods when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation per policy.
• Support care coordination and discharge planning.
