Job summary

ChenMed LLC seeks an Associate Clinical Director to directly supervise and train primary care providers in an assigned center, aiming to maximize core model execution, improve clinical quality, efficiency, outcomes, and clinician/patient satisfaction.

The role includes leading weekly SH and TCM, mentoring PCPs, coordinating with hospitalists, and supporting recruitment and training across centers. Travel locally, regionally, and nationally as needed for training and sessions.

Qualifications

  • MD/DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • Current, active MD licensure in the state of employment.
  • Board certification in the specialty; ongoing maintenance of certification via MOC, CME, or retesting as required.
  • Current DEA number for schedule II–V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.

Responsibilities

  • Provide independent care for older adult patients with acute and chronic illnesses, including assessment, diagnosis, treatment, education, health promotion, care coordination, and documentation.
  • Seek specialist consultation when needed while remaining responsible for detailed patient care.
  • Engage with hospitalists for patients admitted to hospital, regardless of hospitalist affiliation.
  • Lead the care team (medical assistant, care facilitator, care coordinator) for patients able to visit the office, and coordinate with transitional care teams for patients in hospital, SNF, LTC, or homebound.
  • Lead weekly Super Huddle (SH) and Transforming Care Meeting (TCM) and support center clinical leadership when absent, following guidance from Market Chief Medical Officer.
  • Monitor daily center census and jointly account for center outcomes, filling in for clinical leadership needs as required.
  • Actively participate in recruiting patients for the center and interview additional providers for the company.
  • Manage, mentor, and coach PCPs to deliver outstanding clinical outcomes, sample PCP huddles, and partner with Center Operations Director/Market General Manager to improve financial performance and increase center membership.

Responsibilities

  • Minimum one year of clinical experience in geriatric, adult, or family practice setting (lead PCP experience preferred).
  • Board certification (or eligibility) in the specialty; ongoing maintenance of certification via MOC, CME, or retesting as required.
  • Current DEA number for schedule II–V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other company applications.
  • Willingness to travel locally and regionally as needed, and nationally for initial training and occasional recruiting or training sessions.
  • Fluency in English, both written and verbal; additional language proficiency may be required for certain centers.
Pay

$238,832 – $341,189 salary
