Job summary

Ashby Ponds by Erickson Senior Living is seeking an Assisted Living Manager to oversee the success of the Assisted Living Neighborhoods. You will partner with residents, families, and staff to support social services and ensure high-quality care and service.

You will lead holistic, person-centered assessments and care planning, perform pre-residency assessments, coordinate services, and work with interdisciplinary teams. State-specific management certification and ADC/ADCP preferred.

Qualifications

• Minimum of 1-year related experience and/or training in Long-term care/assisted living preferred.
• Experience with the elderly and understanding of the dementia process and its impact on daily living and activity programming.
• Knowledge of geriatric population and aging process, including physical, psychological and social needs.

Responsibilities

• Leading a holistic, person-centered approach to assessment and service/care planning.
• Conducting pre-residency assessments and screening of potential residents and making recommendations for an appropriate level of residency.
• Evaluating psychosocial and ADL needs of residents and coordinating care and services as needed.
• Working with the interdisciplinary team to communicate with residents and families regarding changes in the plan of care and residents' condition.
• Providing individualized psychosocial support to residents and their families via family meetings or support groups and assisting through the transition process to a higher level of care.
• Partnering with Independent Living and Post-Acute Social Workers to develop support groups and educational programming opportunities.
