Job summary

VMP Healthcare & Community Living in West Allis, WI is seeking an Assisted Living Care Coordinator to join our team. You will assist in managing and directing the care and supervision of residents with facility staff, ensuring well-being and quality of life.

CBRF certification is highly preferred; experience in assisted living is valued. The role requires coordination of schedules, intake, ISPs, and collaboration with administrators and nurses.

Qualifications

• 3+ years’ experience in assisted living or nursing facility.
• CBRF certifications preferred (Fire Safety, First Aid, Medication Administration, Standard Precautions, and diabetic training).

Responsibilities

• Act as a positive role-model and leader within the facility.
• Schedule resident appointments and accompany residents to appointments.
• Review resident charts and medical records; ensure quality assurance.
• Ensure care plans are followed and daily shift duties are completed.
• Act as liaison between residents, families and staff.
• Assist with employee onboarding and facility tours & marketing.
• Manage resident intake and admissions.
• Assist with ISP scheduling and care coordination meetings.
• Participate in manager on-call rotation.

VMP Healthcare and Community Living is looking for a Care Coordinator to join our team!

This position is based in our Assisted Living facility, Palmer House. In conjunction with our Administrator and Nurse Supervisors, you will assist in managing and directing the care and supervision of residents provided by facility staff. This role will contribute to the overall well-being of the residents, staff, and the facility. CBRF certification (or ability to obtain within 60 days of hire) is highly preferred as the ideal candidate would be able to fill in for open RA shifts if needed.

Responsibilities of this role include:

• Scheduling resident appointments
• A ccompanying residents to their appointments as needed
• Reviewing resident charts & medical records; quality assurance
• Ensuring careplans are followed and daily shift duties are completed
• Acting as a liaison between residents, families, and staff
• Assisting with employee on-boarding
• Assisting with facility tours & marketing as needed
• Managing resident intake & admissions
• Assisting with ISP scheduling, attending care coordination meetings, and drafting ISPs
• Participating in manager on-call rotation

Knowledge & Abilities:
