Job summary

The Waterford at Fitchburg, a Sonida Senior Living community in Wisconsin, is seeking an Assistant Wellness Director to support the Wellness Director and care team. You will provide clinical leadership, monitor resident needs, and ensure compliance with policies and regulations.

You will supervise direct-care staff, implement nursing policies, communicate expectations, assess residents, and promote compassionate, person-centered care while maintaining safety and quality.

Qualifications

• Two years of professional experience in a senior living or healthcare setting.
• At least two years of supervisory or management experience with direct-care staff.
• Strong communication, organization, and leadership skills.
• A commitment to resident-centered care and service excellence.
• Ability to work collaboratively with residents, families, team members, and leadership.

Responsibilities

• Support the implementation and ongoing evaluation of nursing policies and procedures in accordance with standards and regulations.
• Communicate policies, procedures, and expectations clearly to nursing and care staff.
• Conduct resident assessments and assist with determining levels of care and service.
• Promote a culture of compassionate, person-centered care and ensure staff support.
• Monitor overall quality of resident care, safety, and staff coverage.
• Demonstrate knowledge of Clinical Quality Indicators to promote positive outcomes.
• Review daily reports and follow up on significant changes in resident condition or events.

About The Role

As the Assistant Wellness Director, you will work closely with the Wellness Director and care team to support the delivery of safe, compassionate, and high-quality resident care. You will help provide clinical leadership, support team members, monitor resident needs, and ensure compliance with company policies and applicable state and federal regulations.

Key Responsibilities Include

• Support the implementation and ongoing evaluation of nursing policies and procedures in accordance with current standards of practice, company philosophy, and applicable state and federal regulations.
• Communicate policies, procedures, and expectations clearly to nursing and care staff while supporting consistent implementation.
• Conduct resident assessments and assist with determining appropriate levels of care and service for residents and their families, as directed.
• Promote a culture of compassionate, person-centered care and ensure residents receive appropriate services and staff support.
• Monitor the overall quality of resident care, including cleanliness, staffing coverage, resident safety, and maintenance of residents' physical, mental, and psychosocial well-being.
• Demonstrate knowledge of and support the use of Key Clinical Quality Indicators to promote positive resident outcomes.
• Review daily 24-hour reports and follow up on significant changes in resident condition, incidents, injuries, transfers, discharges, medication errors, potential abuse or neglect, loss of resident property, and resident or family concerns.
• Ensure significant changes in resident condition and other reportable events are appropriately documented and communicated in accordance with company policies and state regulations.
• Partner with the Wellness Director and leadership team to identify opportunities for improvement and promote a positive, collaborative culture among care team members.
• Serve as a visible and supportive leader who helps team members succeed while keeping resident care and safety at the center of all we do.
