Job summary

Sheppard Pratt in Maryland seeks a program supervisor to oversee the Segue Residential Transition Program, coordinating care and supervising day-to-day in-reach and service coordination teams.

The role involves clinical assessments, treatment planning, and collaboration with a multidisciplinary team to help clients transition from hospital to community living, with emphasis on compliance and quality improvement.

Qualifications

• A high school diploma or GED with 5 years of experience.
• Associates degree with 4 years of experience.
• Bachelor’s degree with 2 years of experience.
• CPRP certification by or willingness to obtain.

Responsibilities

• Overseeing in-reach processes, ensuring services are authorized and compliant with guidelines.
• Supervising staff and ensuring onboarding, training, and development.
• Assisting in development and management of the program budget, policies, procedures, and quality improvement systems.
• Maintaining licensing, accrediting, and funding standards, and implementing corrective action plans as needed.
• Conducting comprehensive clinical assessments, developing treatment plans, and implementing interventions for individuals with serious mental illness.
• Supporting clients involved in the forensic system, ensuring compliance with conditional releases and probation requirements.
• Developing strategies to support transition from hospital stays to community living.

Job description

Shift: Day shift| Job Location Type: In Person
The Segue Residential Transition Program assists with helping individuals transition out of state-run psychiatric hospitals. Segue clients are clinically stable and ready to leave the hospital. The services provided assist clients with learning how to manage their illnesses and develop independent living skills.
What to expect.
You will oversee program development, coordinate care, and supervise staff with specific focus on the day-to-day operations of the in-reach and service coordinator teams.
Supervisory responsibilities include:

• Overseeing all in-reach processes, ensuring services are authorized and compliant with guidelines.
• Supervising staff and ensuring effective onboarding, training, and development.
• Assisting in the development and management of the program budget, policies, procedures, and quality improvement systems.
• Maintaining compliance with licensing, accrediting, and funding standards, and implementing corrective action plans as needed.

Client-facing responsibilities include:

• Conducting comprehensive clinical assessments, developing treatment plans, and implementing effective interventions for individuals with serious mental illness, including those with co-occurring disorders.
• Supporting clients involved in the forensic system, ensuring compliance with conditional releases, probation requirements, and collaboration with mental health courts.
• Developing and implementing strategies to support individuals with mental illness in transitioning successfully from long-term hospital stays to community living.

The pay range for this position is $56,560 minimum to $67,897.60 maximum. Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.

  • An associates degree and 4 years of experience.
  • A bachelor’s degree and 2 years of experience.
• A driver’s license with 3-points or less and access to an insured vehicle during work hours.
