Job summary

Allegheny County Housing Authority in McKees Rocks, PA, is seeking an Assistant Director of CBCM to lead community-based care management, health navigation, resident services and the Booker Center. This full-time, 35-hour/week role reports to the Director and oversees staff, coordinates with internal departments and helps reduce barriers related to social determinants of health.

Qualifications

• Master’s degree in community or social services with 5 years of experience.
• Licensure (LSW, LCSW, LPC) preferred.
• Valid PA driver's license and access to a personal vehicle.
• Proficient in Microsoft Outlook, Word, and Excel.

Responsibilities

• Manage the CBCM coordinators, Health Navigators, ROSS Coordinators and the Booker Center.
• Communicate with a multi-disciplinary leadership team
• Be available for consults
• Conduct monthly 1:1’s with staff
• Assist in stakeholder reporting
• Distribute referrals in a timely manner
• Provide guidance and coaching to new hires
• Implement programmatic changes based on need
• Regularly review shift reports/daily contacts
• Perform wellness checks as needed
• Coordinate and supervise interns from local universities
• Keep the Director informed of program status, operational issues, staffing matters, and emerging risks through regular written and verbal reports, and prepare reports and data as requested
• Complete other duties as assigned by the Director and/or Executive Director
