Job summary

WellBridge Healthcare Fort Worth is seeking an Assessment Specialist RN for PRN scheduling to support comprehensive patient assessments and referrals in a psychiatric care setting.

You will conduct initial evaluations, document symptoms and needs, and coordinate with physicians for levels of care, ensuring timely admissions and follow-up. Strong customer service and clinical assessment skills are essential in this role.

Qualifications

• Associate's Degree in Nursing or Master's in social work/counseling required.
• Current clinical, social work or RN license as required by state regulations.
• CPR Certification and CPI training within 30 days.

Responsibilities

• Facilitates response to a request for services, including the initial response, assessment, and referral process, designation of level of care, admission into services, and follow-up.
• Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning and needs and informs MD for LOC determination.
• Demonstrates excellent customer service in assisting patients, physicians, visitors, and coworkers promptly and courteously.
• Maintains preadmission and admission paperwork with accuracy, and completes all pre-certification with insurance companies.
