The role requires understanding of Medicaid and behavioral health, with a focus on compassionate, compliant care coordination. Remote with field visits and a supportive, mission-driven team environment.

Qualifications

• Bachelor's degree in social work, counseling, psychology, or RN/LSW.
• Minimum 1 year post-degree experience with chronic conditions or 3 years with severe mental illness.
• Knowledge of Medicaid and behavioral health systems.

Responsibilities

• Conduct in-home assessments to determine eligibility for programs.
• Develop personalized care plans and connect clients with services.
• Document all interactions and maintain records per policy.
• Maintain confidentiality and comply with CareStar policies.
