CareStar, Inc. in Ohio is seeking an Assessment Specialist for the Ohio Home Care program.

You will conduct visits to assess eligibility, develop personalized care plans, and connect individuals with essential services across Clark, Montgomery, Greene and Miami Counties while working remotely with field visits. Ideal candidates hold an active RN, LSW, LISW or LPN license and have 12 months of home and community-based services experience.

Qualifications

• Active License as RN, LSW, LISW or LPN.
• 12 months experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, Home Health Care and Ohio Home Care waiver programs desirable.
• Adheres to the CareStar Rules and Policies.

Responsibilities

• Completes visits to assess eligibility for specified program and conducts safety assessments.
• Documents all applicant and case-related contacts and interventions in the applicant's record per policy.
• Processes Program Eligibility Assessment Tool (PEAT) or other tools and related documents.
• Provides assistance with community resource referrals as appropriate.
• Identifies and reports high-risk situations as required per policy.

Key Responsibilities

• Processes specific Program Eligibility Assessment Tool (PEAT) or other tools and all related documents per agency and CareStar policy.
• Provides assistance with community resource referrals as appropriate.
• Identifies and reports high-risk situations as required per agency and CareStar policy.

Minimum Qualifications

• Active License as RN, LSW, LISW or LPN.
• 12 months experience in home and community-based services within the past 5 years.
• Knowledge of Medicare, Medicaid, Home Health Care and Ohio Home Care waiver programs desirable.
• Adheres to the CareStar Rule in performance of job responsibilities.
