Job summary

Westpark Springs, part of Lifepoint Health, is seeking an Assessment Coordinator to join our team. You will assess walk-in and phone callers, determine services, and schedule initial psychiatric assessments for treatment seekers.

In this role, you will collaborate with the Care Coordination Department to streamline admissions, document calls, coordinate intake paperwork, and support the team with data entry and forms management. A strong commitment to compassionate care is essential.

Qualifications

• High School Diploma or GED required (preferred).
• Basic Life Support certification within 30 days of hire.
• Experience in a psychiatric facility with adults/geriatrics preferred; strong assessment skills.

Responsibilities

• Assess walk-ins and calls to determine services and schedule initial psychiatric assessments.
• Answer inquiries about treatment and services; present options and set appointments.
• Collaborate with business office to ensure efficient admissions and referrals as needed.
• Provide clerical support to Care Coordination Manager and enter data into the EHR.
• Maintain stock of forms and supplies for Care Coordination.
• Check in belongings, process consent forms, coordinate admission paperwork.
• Schedule assessments, orient to units, assist with discharges and send docs to referral sources.
• Perform other duties as assigned.

How you'll contribute

An Assessment Coordinator who excels in this role:

• Assess walk‑in and phone callers to determine appropriate services or referrals and schedule initial psychiatric assessments for individuals seeking treatment for substance abuse, dual diagnosis, or emotional disorders.
• Answer inquiry calls regarding treatment and facility services, present treatment options, document calls, and set appointments for callers seeking treatment.
• Collaborate with business office staff to ensure an efficient admission process and refer or triage patients to other treatment sources when appropriate.
• Provide clerical support to the Care Coordination Manager and team, including data entry into the electronic record system.
• Maintain an appropriate stock of forms and supplies to support the Care Coordination Department.
• Check in patient belongings, process consent forms, and coordinate admission paperwork.
• Schedule assessment appointments, orient patients to units and outpatient programs, assist with discharges, and send documentation to referral sources within required timeframes.
• Perform other duties as assigned.

What we're looking for

• Education: High School Diploma or GED preferred.
• Basic Life Support certification is required within 30 days of hire.
• Experience: Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred. Must have strong clinical assessment skills.
