Job summary

Triangle Springs is a 77-bed behavioral health hospital in Raleigh seeking an Assessment Coordinator to schedule initial psychiatric assessments for individuals seeking treatment for substance abuse, dual diagnosis, or emotional disorders. You will assess callers, document calls, and set appointments to support an efficient admission process.

This full-time role requires a High School Diploma or GED, BLS within 30 days, and prior psychiatric healthcare experience with strong clinical assessment

Qualifications

• High School Diploma or GED preferred.
• Basic Life Support certification required within 30 days of hire.
• Experience in a psychiatric healthcare facility with adults and geriatric patients; strong clinical assessment skills.

Responsibilities

• Assess walk‑in and phone callers to determine services or referrals and schedule initial psychiatric assessments.
• Answer inquiry calls, present treatment options, document calls, and set appointments.
• Collaborate with business office to ensure efficient admissions and triage to other sources when appropriate.
• Provide clerical support to Care Coordination Manager and team, including data entry.
• Maintain stock of forms and supplies for the Care Coordination Department.
• Check in patient belongings, process consent forms, and coordinate admission paperwork.
• Schedule assessment appointments, orient patients to units and programs, assist with discharges, and send documentation to referral sources.
• Perform other duties as assigned.

How you'll contribute

An Assessment Coordinator who excels in this role:

• Assess walk‑in and phone callers to determine appropriate services or referrals and schedule initial psychiatric assessments for individuals seeking treatment for substance abuse, dual diagnosis, or emotional disorders.
• Answer inquiry calls regarding treatment and facility services, present treatment options, document calls, and set appointments for callers seeking treatment.
• Collaborate with business office staff to ensure an efficient admission process and refer or triage patients to other treatment sources when appropriate.
• Provide clerical support to the Care Coordination Manager and team, including data entry into the electronic record system.
• Maintain an appropriate stock of forms and supplies to support the Care Coordination Department.
• Check in patient belongings, process consent forms, and coordinate admission paperwork.
• Schedule assessment appointments, orient patients to units and outpatient programs, assist with discharges, and send documentation to referral sources within required timeframes.
• Perform other duties as assigned.

What we're looking for

• Education: High School Diploma or GED preferred.
• Basic Life Support certification is required within 30 days of hire.
• Experience: Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred. Must have strong clinical assessment skills.
