Job summary

Wellvance Community Mental Health Authority is seeking an ACT Case Manager to work on-site in northern Michigan. You will join the Assertive Community Treatment team to help individuals with severe mental illness manage symptoms, integrate into the community, and achieve the highest possible quality of life.

The role requires a bachelor’s degree in a human services field and QMHP/QIDP/CMHP licensure (or Michigan licensure). Flexibility to travel within the service area is expected.

Qualifications

• Bachelor’s degree in a human services field.
• One year of experience in the mental health field.
• QMHP, QIDP, CMHP, or limited/full Michigan licensure in your field.

Responsibilities

• Provide ACT services to individuals with severe mental illness.
• Help clients manage symptoms, integrate into the community, and achieve higher quality of life.
• Follow the ACT fidelity model and Wellvance policies.
