RHA Health Services, LLC is hiring an IDD Qualified Professional/PA/Hearing Deaf Services (ASL) in Pittsburgh. The QP guides person-centered plans, coordinating a cross-disciplinary team to address health, psychological, and independence goals.

Qualifications

• Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required.
• Master’s degree a plus.
• Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required.
• 1-year experience if paired with Master’s degree; must be 18+ and fluent in ASL.

Responsibilities

• Coordinate IDT meetings and the ISPs.
• Assist individuals to identify and communicate requests and needs.
• Supervise and coordinate person-centered plans and monitor outcomes.
• Initiate periodic reviews and adjustments as needs change.
• Review assessments and ensure plans meet person goals.
• Write and supervise the person-centered plan.
• Oversee residential activities, safety, and community integration.
• Visit vocational centers and residential sites to ensure plan implementation.
• Report to Administrator and coordinate with colleagues to improve workflows and performance.
• Monitor quality assurance and state survey improvements.
• Serve as liaison between individuals and families/guardians for services.
