Hire Consulting Services seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization operating in a PCMH model. You will collaborate with physicians, PAs, ARNPs, and nursing staff to deliver high-quality care while supporting education, care coordination, and patient empowerment across a diverse patient population.

Qualifications

• Graduate of an ARNP program and licensed as RN/ARNP in Washington.
• Prescriptive authority for WA required.
• Broad primary care clinic experience preferred.
• Strong knowledge of the nursing process: assessment, planning, implementation, evaluation.
• Experience with minor surgical techniques preferred.
• Ability to use evidence-based data to guide decisions.

Responsabilités

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Complete fee slips with appropriate diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters/year full-time).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor population health by panel.
• Educate patients for self-management and discharge planning.
• Advocate for patients by explaining procedures and treatment plans.
