Job summary

Tom Trese Networking LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare team. This permanent, full-time position operates within the Patient Centered Medical Home model and collaborates with physicians, ARNPs, PAs, and nursing staff to deliver high-quality care.

Key responsibilities include advanced clinical assessments, care coordination, patient education, and documentation.

Qualifications

• Must be a graduate of an accredited ARNP program.
• Current Washington RN and ARNP licensure required.
• Prescriptive authority for WA required.
• Experience in a broad primary care clinic setting required.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patient health.
• Maintain timely chart documentation and accurate coding.
• Provide care coordination through daily huddles as part of PCMH.
