Job summary

Good Shepherd Rehabilitation seeks a Physical Therapist to facilitate restoration of function and prevention of disability for patients with rehabilitation needs by planning and administering medically prescribed physical therapy.

Responsibilities include assessing patient condition, setting goals with the health-care team, documenting interventions, and ensuring safety and quality of care while adhering to licensure and regulatory standards.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Previous outpatient experience or similar setting preferred.
• Physical Therapist license required.
• Basic Life Support / CPR required.

Responsibilities

• Assess patient condition upon physician order and at regular intervals during shift.
• Integrate patient information to facilitate holistic patient care.
• Set goals, collaborate and communicate with health-care team, patients and families.
• Act on physician’s order and patient need in a timely manner.
• Document interventions and responses.
• Revise care plans based on patient response.
