Responsibilities

• Assess patient condition upon physician order and at regular intervals during shift.
• Integrate patient information to facilitate holistic patient care.
• Set goals, collaborate with health-care team, patients and families.
• Act on physician''s order and patient need in a timely manner.
• Document interventions and responses.
• Revise care plans based on patient response.
• Monitor delivery of care and identify process improvements.
