Job summary

Good Shepherd Rehabilitation is seeking a licensed Physical Therapist to evaluate, plan, and administer individualized therapy programs to restore function and relieve pain in patients. You will document exams, adjust treatment plans, and collaborate with physicians and care teams.

Responsibilities include supervising therapy students, ensuring compliance with licensure and safety standards, conducting patient education, and maintaining accurate chart notes.

Qualifications

• Bachelor's Degree in Physical Therapy is required.
• Master's Degree is preferred.
• Physical Therapist license is required.
• Basic Life Support / CPR certification is required.

Responsibilities

• Plan, prepare and carry out individually designed programs of physical treatment to restore physical functioning and alleviate pain.
• Perform and document an initial exam to identify problems and determine a diagnosis.
• Evaluate effects of treatment and adjust plans for maximum benefit.
• Supervise, train and assess therapy students during onsite clinical affiliations.
• Confer with patient and care team to plan, implement and assess intervention programs.
• Record prognosis, treatment and progress in charts and electronic systems.
• Discharge patients when goals are reached and coordinate follow-up care.
