Job summary

Griffin Hospital is seeking an experienced NP specializing in wound care to join our team on site in Derby, CT. The role requires 1+ year NP practice and 5+ years RN experience, with WOCN certification preferred.

You will manage wound debridement, topical therapies, and coordinate related services to optimize healing. You will educate patients and collaborate with PCPs, CM, and referring physicians, providing updates on patient status and aligning care with ADA guidelines.

Qualifications

  • Must be a registered nurse (RN) and licensed Nurse Practitioner (NP).
  • Minimum 1 year NP experience and 5 years RN experience.
  • WOCN Certification preferred.
  • EOE/Minorities/Females/Vet/Disabled.

Responsibilities

  • Debridement of wounds by sharp, enzymatic, autolytic or surgical methods as required in the patient’s visit.
  • Application and management of topical wound care and related products.
  • Direct and recommend related services to improve wound healing outcomes (labs, radiology, vascular tests, referrals, home health care, VAC device, orthotics).
  • Interpret diagnostic tests and recommend appropriate follow ups.
  • Recommend medications or DME to support wound healing within scope and supervising physician agreement.
  • Perform diabetic foot ulcer risk assessments per ADA guidelines.
  • Educate patients on wound management and therapies.
  • Educate PCPs/CM/referring physicians on current wound care practices and updates to their status.
  • Act as resource for decisions, planning, and policies affecting patient health.
  • Collaborate with staff to identify needs and help develop plans to meet them.
  • Other duties as assigned

Job Description/Specific Duties:
  • Debridement of wounds by sharp, enzymatic, autolytic or surgical methods able to be safely completed in the place of patient visit and all associated procedures to complete the debridement.
  • Application and management of topical wound care and associated wound management products.
  • Direction and recommendation of associated services to improve wound healing outcomes. This includes but is not limited to labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
  • Interpretation of diagnostic tests and recommendation of appropriate follow up.
  • Recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.
  • Diabetic Foot Ulcer risk assessment for Members with Diabetes, as per the ADA guidelines
  • Education of patients as to the topical and systemic management of their wounds.
  • Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
  • Provision of regular updates to the PCP, CM, or referring Physician on the status of their patient's systemic conditions associated with wound care.
  • Acts as a resource person for decisions, planning, and policies affecting the health of residents.
  • Collaborates with staff to identify customer needs and to help develop plans to meet these needs.
  • Other duties as assigned
Requirements
  • Registered nurse (RN) preparation and Nurse Practitioner (NP) license required.
  • Minimum of 1-year experience as an NP and 5 years' experience as an RN.
  • Certification from the Wound Ostomy and Continence Nursing Certification Board preferred.
