• Credentialing and privileging required

Responsibilities

• Provides follow-up and health maintenance care to patients based on standards of care and best practices.
• Organizes and presents historical and physical data to the healthcare team to identify medical problems and determine diagnostic or therapeutic procedures.
• Works within a multidisciplinary team to provide diagnostic, therapeutic and preventive care.
• Instructs and counsels patients on health promotion and maintenance.
• Executes diagnostic and therapeutic procedures as indicated.
• Obtains complete medical history and performs comprehensive and focused physical examinations.
• Maintains complete and accurate records of patient contacts (in person, virtually or by telephone).
