Job summary

AdventHealth Primary Care+ seeks Advanced Practice Providers to join a team-based primary care model in Florida. The role emphasizes whole-family care, continuity with patients, and a collaborative environment that balances high-quality care with work–life balance.

Providers will work with Care Teams to deliver preventive care, chronic disease management, and long-term patient relationships in a modern, well-supported clinic. A strong focus on collaboration and patient-centered care is outlined.

Qualifications

• APRN or PA-C licensure in Florida is required.
• Experience in primary or family medicine is preferred.
• Strong skills in preventive care and chronic condition management are expected.

In this role, you will work closely with other Advanced Practice Providers and Care Teams to deliver comprehensive primary care in a modern, well-supported clinic focused on preventive care, chronic disease management, and long-term patient relationships.
