Job summary

Lifespan Therapy is seeking an experienced Home Health Physical Therapy Assistant (PTA) to execute individualized plans of care under the supervision of a Physical Therapist. You will provide skilled PTA care in patients' homes, promoting mobility and independence.

You’ll schedule visits flexibly using our app, document progress, and communicate with the supervising PT to ensure coordinated care. California licensure, CPR/BLS, and clean driving record are required.

Qualifications

• Active California PTA license is required.
• CPR or BLS certification is required.
• Driver’s license and auto insurance are required.
• Physical and TB test completed within the last year.
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PTA Care: Implement individualized plans of care.
• Perform Therapeutic Interventions: Focus on mobility, strength, balance, and fall prevention.
• Report Patient Progress: Document responses and coordinate with supervising PT.
• Manage Your Caseload: Schedule visits and cadence per plan of care.
