Phoenix Houses of California is seeking an AOD Counselor to provide individualized treatment coordination for clients in residential/outpatient settings in Victorville. You will manage caseloads, integrate medical care and substance abuse treatment, and support transition back to the community.

Responsibilities include coordinating assessments, treatment plans, group counseling, crisis intervention, and documentation while collaborating with a multidisciplinary team and families to foster

Qualifications

• High school diploma or equivalent; must be registered as AOD Counselor with DHCS.
• Minimum one year of related field experience.
• Bachelor's degree preferred.
• Must maintain required credential during employment.
• TB test clearance required yearly.

Responsabilités

• Coordinate initial assessment, treatment plans, and service goals with other plans.
• Provide individual counseling services.
• Facilitate evidence-based group counseling for assigned cohorts.
• Provide crisis intervention as needed.
• Submit documentation (progress notes, assessments, treatment plans) timely.
• Maintain and file clinical records and discharge summaries.
• Collaborate with multi-disciplinary team and families to support recovery.
• Attend staff meetings and participate in program activities.
