Job summary

Mass General Brigham is seeking a Clinical Pharmacist to optimize health outcomes for high-risk patients within its care management teams. You will deliver direct patient care, perform comprehensive medication reviews, and align pharmacy workflows with quality initiatives.

Responsibilities include counseling, adherence interventions, transitions of care, medication reconciliation, and collaboration with physicians using Epic and other EHRs.

Qualifications

• PharmD degree from an accredited college or university.
• Current license to practice in MA; NH license within 90 days.
• Board certification in pharmacotherapy (BCPS/BCACP/BCGP) recommended.

Responsibilities

• Collaborate with Care Management team to deliver pharmacist support to a high-risk patient population.
• Provide counseling to patients and use motivational interviewing to promote behavior change.
• Assess adherence barriers and develop patient-centered plans to improve medication adherence.
• Address affordability concerns and optimize drug therapy; leverage prescription assistance programs.
• Perform Comprehensive Medication Management reviews for enrolled patients.
• Conduct transitions of care medication reconciliation during transitions of care.
• Identify high-risk medication issues to improve post-discharge outcomes and support health system goals.
• Document patient encounters and update medication lists in Epic and other EHRs.
• Participate in pharmacy-led quality improvement projects across value-based contracts.
