The University of Kansas Health System is seeking an Ambulatory RN Care Coordinator for the Spine Center. This part-time position supports physicians during clinic sessions, coordinates patient care, educates patients, and documents results in the EMR.

Responsibilities include triage, patient flow management, and ensuring the care plan is understood by patients and families. Experience in ambulatory clinics and strong communication are preferred.

Qualificações

• High School Graduate
• Bachelor Degree Nursing
• 2 or more years of RN experience.

Responsabilidades

• Manage patient flow, triage and direct patient care.
• Liaison of communication between front desk staff, nursing staff, physicians, and clinic manager
• Develop expected patient outcomes and coordinate resource utilization with provider to set patient goals.
• Provide nursing support/coordination to optimize patient outcomes and quality of life.
• Support patient self-management of disease.
• Coordinate continuity of patient care post-discharge and ER visits.
• Manage high risk patient care and complex comorbidities with a registry.
• Educate patients and families about health needs and medications.
• Document assessments and plans in EMR and communicate results to physicians.
