• Develop expected patient outcomes and coordinate resources with providers.
• Provide nursing support and education to improve patient outcomes and quality of life.
• Educate patients and families about plan of care and medications.
• Document assessments, plans, and progress in EMR accurately.
• Maintain patient confidentiality and comply with standards.
