Qualifications

• Master's Degree required for the role.
• Experience in healthcare or community services preferred.
• Independent License Eligible for LICSW/LMHC/LMFT as defined.
• Bilingual skills such as Spanish are optional.

Responsibilities

• Provides social work assessment and counseling and refers patients to community resources.
• Evaluates patient and family support, economic status, and needs to access services.
• Conducts interviews to assess issues and support adjustment to illness or treatment.
• Coordinates care with health care professionals to plan ongoing care.
• Provides clinical consultation on psychosocial issues within the team.
• Supports Baystate Health mission and service standards.
• Utilizes health care networks to optimize resources and patient satisfaction.
• Plans and coordinates patient care conferences and follow-up actions.
• Enters data into EMR and maintains accurate records for legal purposes.
• Develops relationships with community agencies and disseminates updates.
• Maintains professional conduct and contributes to interdisciplinary care.
• Performs other job-related duties as assigned; may supervise interns.
• May supervise social work interns and/or licensed social workers for licensure hours.
