Baystate Health seeks an Integrated Behavioral Health Medical Social Worker- Baystate Medical Center to join the ambulatory social work team. The SW-I will address psychosocial and behavioral health needs, guiding patients and families to resources to improve continuity of care and outcomes.

This full‑time role supports Monday–Friday scheduling, with a focus on collaborative care within a multidisciplinary team and documentation in the EMR to ensure coordinated services across the care continuum.

Qualifications

• MSW degree required; licensure encouraged within six months of eligibility.
• Minimum one year in health care or social services; internship in health care may be considered.
• Spanish/bilingual skills optional.
• Knowledge of community services and programs; strong communication skills.

Responsabilités

• 1) Perform social work assessment and counseling with referrals to housing, education, welfare and other services.
• 2) Assess patient/family support, economic status and access to resources; report abuse risks as needed.
• 3) Conduct interviews to identify psychosocial issues and coping needs.
• 4) Coordinate input to formulate a documented plan of care for continuity.
• 5) Provide clinical consultation on psychosocial and care barriers within the team.
• 6) Support department mission and Baystate standards in daily operations.
• 7) Leverage care networks; coordinate with inpatient social workers for smooth transitions.
• 8) Plan and coordinate patient care conferences with follow-up interventions.
• 9) Enter data into EMR; maintain records; refer to domestic violence resources when appropriate.
