Job summary

UNC Health is seeking a Social Worker – Ambulatory II to provide psychosocial assessment, care coordination, and ongoing support for patients and families in an ambulatory setting in Chapel Hill, NC.

The role requires an LCSW license in North Carolina, with dual licensure (LCSW and LCAS) preferred, and at least three years of broad clinical healthcare experience. 40 hours/week onsite with UNC Health benefits.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Licensed as LCSW in North Carolina.
• Three years of broad clinical healthcare experience required.

Responsibilities

• Assessments: conduct biopsychosocial evaluations to identify needs and barriers.
• Care plans: develop individualized plans with the care team.
• Resource facilitation: connect patients with community services and referrals.
• Education: provide patient education, referrals, and transition planning.
• Documentation & billing: accurate coding of services per requirements.
• Crisis management: provide crisis intervention and support.
• Progress monitoring: track outcomes and adjust care plans.
• Advocacy: ensure access to needed treatments and services.
• Active listening: create a safe, nonjudgmental space for patients.
• Motivational interviewing: support behavior change towards health goals.
• Respect for individuality: tailor interventions to cultural values.
• Building trust: foster confidentiality and professionalism.
• Meetings: participate in problem-solving and quality improvement.
• Training: provide education and develop standard work as needed.
• Consultation: serve as a resource for the care team.
