Job summary

Rush University Medical Center in Chicago, IL seeks a Social Work Care Manager I to provide patient-centered care across clinical teams. You will coordinate psychosocial services, perform assessments, and develop care plans to support patients with complex needs.

The role emphasizes collaboration, education, and outcome-driven practice within ambulatory care settings. You will maintain regulatory documentation, contribute to program development, and participate in supervision and staff education

Qualifications

• Master’s degree in Social Work from an accredited university required.
• Current license in Illinois as a Social Worker, LSW OR proof of a pending license obtained within 90 days of hire required.
• Experience related to psychosocial issues, crisis management, conflict resolution, person-centered planning, and care transitions required.
• Be willing to co-locate in the outpatient clinic when needed.
• Proficient and functional knowledge of Microsoft Office Suite.
• Process improvement skills, ability to perform tasks independently, prioritize workload, problem-solve, and analyze data.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Uses MHN portal and value-based care EMR reports to locate patients needing outreach from a psychosocial perspective.
• Assesses patient physiology, psychosocial, financial, cultural, and familial factors, coordinating psychosocial services.
• Develops a psychosocial plan of care and acts as a change agent for complex cases.
• Aligns care plans with available resources and promotes cost-effective, high-quality care.
• Participates in program development, implementation, and evaluation in RUMG clinics.
• Provides crisis intervention when needed.
• Delivers educational materials to patients and families in accessible language and formats.
• Measures care outcomes using defined quality metrics.
• Maintains a proactive, patient-centered approach to meet department goals and process improvements.
• Documents per regulatory guidelines and communicates with treatment teams about progress and discharge plans.
• Organizes clinical supervision for social work staff and oversees ongoing education needs.
• Promotes education about the care management role across the organization.
• Ensures policies align with regulatory guidelines; updates committees as needed.
• Other duties as assigned.
