Job summary

Rush University Medical Center in Chicago seeks a Licensed Clinical Social Worker to provide social work services to patients and community members, connecting those served to Rush and community resources. The role supports relevant programming for this population and may represent the department in meetings and presentations.

The position requires a Master’s Degree in Social Work and an Illinois LCSW license, with at least two years of healthcare or related social services experience.

Responsibilities

• Works directly with patients and their families to address unmet needs and provide social work services to help achieve care goals.
• Carries out comprehensive social work assessments by phone or in-person to develop a plan of care for the patient and/or family.
• Connects patients and families to relevant resources, both at Rush and in the community.
• Provides care management and/or support programming as outlined by Manager.
• Documents all client interactions in Epic, LCM software and/or other systems as indicated.
• Maintains relationships with referring Rush clinics through timely communication.
• Represents the department at meetings, conferences or other engagements as needed.

Pay Range: $29.36 - $47.79 per hour

Summary

The Licensed Clinical Social Worker is a key member of the Rush University System for Health community. This position provides social work services from a variety of referral sources and connects those patients and community members to relevant resources both internal to rush and in the community. This position supports relevant programming for the aforementioned population also and may serve as a representative of their department in meetings and presentations. This position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Required Job Qualifications:

• Master’s Degree in Social Work.
