Pay Range: $10.00 - $500.00 per hour

The Nurse Care Manager is an Illinois licensed Registered Nurse who provides care coordination services to Rush patients through a diverse role as a clinician, educator, care coordinator and advocate. The Nurse is an expert in chronic disease management, prevention, health maintenance, and clinical evaluation of medical and psychosocial needs. Provides timely telephonic outreach to discharged patients in order to better engage them with their own care. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.
