Job summary

Advocate Health is seeking an experienced Registered Nurse for Ambulatory Triage. The role supports telephone triage, patient assessment, and care coordination in a fast-paced setting with clinical judgment and strong communication skills.

The ideal candidate has at least five years RN experience, ambulatory/triage exposure, and proficiency with Epic, MS Teams, and SharePoint. Shift is 7:30 AM–4:30 PM or 8:00 AM–5:00 PM, Monday–Friday, with no weekends and holiday coverage.

Qualifications

• Minimum of five (5) years RN experience preferred.
• Ambulatory and/or telephone triage experience preferred.
• BSN preferred; not required.
• Current RN license in applicable state required; SC licensure process outlined later.

Responsibilities

• Performs telephone triage with diverse patient population.
• Assesses needed care and collaborates with patients to provide clinical solutions.
• Provides comprehensive nursing assessment in an untraditional nursing environment.
• Notifies practices, physicians, and managed care clients of encounters before office opening.
• Notifies leadership of operational concerns.
