Job summary

Advocate Health seeks a Registered Nurse to perform telephone triage for a diverse patient population, assess care needs, and collaborate with patients and care teams in an unconventional nursing setting.

The role requires active participation in team discussions and education planning, with timely communication to practices and leadership about encounters and operational issues. A BSN is preferred and RN licensure in the applicable state (SC licenses may be needed) with BLS per policy.

Qualifications

• Graduate from an accredited School of Nursing required.
• BSN preferred.
• Current RN license in state of practice required; South Carolina license and additional state licenses may be required for clients.
• BLS certification required per policy guidelines.
• Experience in telephone triage, pediatrics and ED triage is desired.

Responsibilities

• Performs telephone triage with diverse patient population.
• Assesses needed care and collaborates with patients to provide clinical solutions.
• Provides comprehensive nursing assessment in an untraditional nursing environment.
• Participates in team conferences and helps assess patient education needs.
• Notifies each practice, physician, and/or managed care client of all encounters with patients, parents or managed care clients prior to office opening.
• Notifies designated leadership of operational concerns.
