Zusammenfassung

Southern New Hampshire Health in Nashua, NH is seeking a Registered Nurse Ambulatory to deliver professional nursing care across Foundation practices, including float pool support. You will work with providers and the care team to ensure safe, evidence-based, patient-centered care in-person and via telehealth and triage.

The role requires ASN, NH RN license, BLS, and 1+ year of nursing experience; ambulatory care is a plus.

Qualifikationen

ASN required; BSN preferred.
NH RN license (or NLC state/MA) and BLS certification required.
1+ year nursing experience; ambulatory care a plus.
Strong knowledge of EMR and patient-centered care.

Aufgaben

Carry out the nursing process across ambulatory and telehealth settings.
Collaborate with physicians and care team to design plans of care.
Administer medications, vaccines, and point-of-care testing.
Educate patients on self-management and wellness.
Document care in EMR and participate in pre-visit planning.
Provide leadership and mentorship to clinical staff and assist with onboarding.
Respond to urgent situations per facility guidelines.
Stay current on competencies and licensure.
